Welcome to the Body Swap Request Form! Here you can submit a request to exchange your broken or damaged BMM Device Body for a new one.
PLEASE ONLY SUBMIT THIS FORM IF YOUR DEVICE BODY IS BROKEN OR DAMAGED.
Cost: Rates may vary, starting at $100.
Forms will be replied to and schedules will be made within two weeks of request.
Spa Day
Welcome to the Spa Day/Repair Request Form! In the form below, please enter what type of BMM Device you would like serviced and what you would like us to particularly focus on. A Spa Day treats most ailments your device may suffer from.
Cost: Rates may vary, starting at $35.
If your device body is damaged, YOU ARE IN THE WRONG PLACE. To fix body issues, please go to the Body Swap Form under Services.
Forms will be replied to and schedules will be made on Mondays and Thursdays.

The members of Captain Chucks have years of Mixology experience and have come together to offer their unique recipes to you! We take the art very seriously and scrutinize every detail in the creation of our flavor profiles, following strict GMP guidelines for consumable products, using only the Highest quality USP Kosher VG/PG and simply the best ingredient flavors available in our vested recipes. We stress the process of creating each profile and spot test every item to ensure that only the best and most consistent product leaves our facility.
